Has anyone ever had to submit a safety plan along with a bid? Anyone willing to share an example of such a thing?

I have had such boilerplate requests on a couple of occasions AFTER winning a contract, but it turns out they are routinely sent to all "contractors" because they apply to people like HVAC, roofing, and other contractors.  I guess you could say that an account with large trees that could topple and injure someone or damage property might be a logical context for such plans, but I never had to actually execute one for a client.
